Dear Former Client of Foundations Therapeutic Services,
I hope this notice finds you well. This notice is to inform you that Foundations Therapeutic Services will be permanently closing on December 31, 2025. While the practice will no longer be operating, I will continue practicing at The Mental Health Co-Op in Palmer, Alaska, where I am a group partner.
If you received care with one of our clinicians over the years, please know that your records will continue to remain confidential and protected in accordance with state and federal laws. As part of this transition, your treatment records will automatically transfer with me to my new practice on November 17, 2025, unless you request to have them transferred to your current provider. If you would like your records sent to your current provider, please submit a written request by November 16, 2025. If you prefer the automatic transfer, no action is required on your part.
It has been a privilege to work with you, and I deeply value the trust you have placed in me. I wish you the very best in your continued journey of growth and wellness.
